Health plan types, explained
Plan names can sound alike. Here is how the most common network types differ in everyday use.
| Plan type | How it works | Referral needed? | Out-of-network care |
|---|---|---|---|
| HMO | Choose a primary care doctor who coordinates your care within a network. | Usually yes | Generally not covered, except emergencies |
| PPO | See any doctor, with lower costs when you stay in network. | No | Covered at a higher cost |
| EPO | Use the network only, but no primary care doctor requirement. | Usually no | Generally not covered, except emergencies |
| POS | A blend of HMO and PPO: a primary doctor plus some out-of-network options. | Usually yes | Partly covered |
| HDHP with HSA | Lower monthly premium and higher deductible, paired with a tax-advantaged savings account. | Depends on network | Depends on network |
Not sure which fits you? We’ll check whether your doctors and prescriptions are covered before you choose.
Why having health insurance matters
One unexpected hospital stay can cost tens of thousands of dollars. Coverage turns an unpredictable bill into a manageable one and makes it easier to get care early, when it works best.
Working with an independent agency adds another benefit: you get a person who explains the fine print, compares carriers, and stays available after you enroll.
